CFC Pediatric Program

Every Wednesday morning the Clearwater Free Clinic focuses on our younger population…uninsured kids! All qualified children… newborns to 18 years of age, who are sick or in need of a physical, are able to come to the Clinic for medical care.

In addition to Wednesday mornings, pediatric appointments are scheduled throughout the week with the Clinic Nurse Practitioner as needed.

  • Sick Visits
  • School Physicals
  • Medications

Patient Qualifications

  • Resident of mid or upper Pinellas County
  • US citizen or Permanent Resident of the US
  • Meet low-income financial guidelines
  • Not have any insurance, including: Medicaid, County Social Services or KidCare

How to Become a Pediatric Patient:

Required Documents 

 

  • Proof of Child’s US Citizenship or lawful permanent residence: Please bring one of the following:
    •  US Birth Certificate – Parent must be listed on birth certificate
    •  US Passport
    •  US Naturalization Certificate
    •  Permanent Resident Alien Card (I-551)
    •  Foreign Passport stamped by the U.S. Government “Processed for I-551″
  • Parent’s Proof Current Address: Example: rent receipt, bank statement, cable or utility bill, junk mail addressed to you
  • Child’s Social Security Card
  • Parent’s Photo ID

Mon – Thurs: 8:45am – 11:30am

CFC Main Building – 707 N Ft. Harrison Ave – Clearwater
Tues: 4pm – 6pm
