At the Clearwater Free Clinic, we’re always looking for ways to make accessing care more convenient for our patients. That’s why we’ve introduced an online medication refill request form—giving you the option to request your refills quickly and easily, any time of day.
Whether you’ve run out of your daily medication or need a refill before your next appointment, our online form is designed to save you time and help us serve you more efficiently. You no longer need to call the Clinic or wait on hold. With just a few clicks, you can submit your request and our pharmacy team will take care of the rest.
Here’s how it works:
Fill out the short form with your name, date of birth, and the medications you need
Submit your request—it’s that simple
Please allow 48 hours for your request to be processed. If there are any issues with your refill, someone from our team will contact you directly.
