Now entering Phase 4, the Clinic is expanding beyond problem-focused care to include prevention and wellness—helping patients stay healthier, longer. From screenings and nutrition support to behavioral health and medication management, this next phase ensures that every patient has a true healthcare home, no matter where they are on their journey.
