  • We are now offering phone and video appointments via telehealth.

  • We must have patient’s correct email address on file to participate in telehealth appointments.

  • Patient must complete all forms sent by the CFC through email.

  • We are filling medications over the phone. Patients must call-in 24 hours to pick-up.

  • If you have an in-person appointment:

    • Observe social distancing by being no closer than 6 feet from other individuals

    • Only bring guests/escorts into the clinic if they are required for physical or cognitive assistance, language translation.

    • Bring and wear your mask upon arrival to our clinic. Please continue to wear your mask throughout your appointment and departure from the clinic.
